Your First Visit Starts Here
Everything you need to know before your appointment at Bright Smiles.

Save time at your first appointment by completing your new patient paperwork ahead of time.
Download the forms below, fill them out, and bring them with you to your appointment. If you prefer, you can complete your paperwork when you arrive at our office.
Your First Visit
We want your first visit to Bright Smiles to be comfortable, informative, and stress-free. Dr. Lori takes the time to get to know you, understand your dental concerns, and discuss your goals for your smile.
Whether you’re coming in for routine care, haven’t visited a dentist in a while, or have a specific concern, we’re here to help you feel comfortable and informed about your dental health.
What Should I Bring to My Appointment?
Please bring a photo ID, your current dental insurance card (if applicable), and a list of any medications you’re taking.
If you’ve completed your new patient forms ahead of time, bring those as well. Having this information ready helps us make your first visit as smooth as possible.
What Can I Expect During My First Visit?
Your first visit gives us an opportunity to get to know you and understand your dental health.
We’ll review your medical and dental history, discuss any concerns you have, and evaluate your oral health. Dr. Lori will explain her findings, answer your questions, and discuss any recommended treatment options.
Our goal is to make sure you understand your dental health and feel confident about your next steps.
What If I Haven’t Been to the Dentist in Years?
It’s never too late to get back on track with your dental health.
Whether it’s been a few years or much longer since your last dental visit, you’re welcome at Bright Smiles. Dr. Lori will take the time to listen to your concerns, assess your dental health, and help you understand your options.
There’s no judgment and no pressure. We’ll focus on where you are today and work together on a plan that makes sense for you.

Mondays, Wednesdays, and Fridays are generally the best days for emergency appointments. Tuesdays and Thursdays tend to be our busiest days, making emergency scheduling more difficult.
